Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

How to Register

  • After you have collected and completed the forms please bring them back to the surgery with 2 forms of identification (ID).
    One of these must contain your address (e.g. utility bill, tenancy agreement, mortgage agreement, bank statement), and the other to have your photograph (eg. driving licence, passport, for children a birth certificate is adequate).
    Until you have submitted your ID you will not be registered at the practice and cannot be seen for any treatment.
  • Please provide your NHS number (10 digit number) on the registration form. This will help us trace your medical records more quickly.
  • You will be allocated a specific GP who has vacancies on their list. If you have a preference please speak to the Patient Services staff member. However, if the doctors list is full, it may not be possible to accommodate your preference.

New Patient Health Check

All new patients aged over 18 are invited to make an appointment for a routine health check with the GP. Where you can discuss any regular medication or any significant medical conditions.
