You do not need proof of address or immigration status, ID or an NHS number.<\/p>\n\n\n\n<p>The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.<\/p>\n\n\n\n<p>Paper forms are still available if you need one.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/gp-registration.nhs.uk\/J81631\/gpregistration\/landing\">Register at our GP surgery<\/a><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Register with a paper form<\/h2>\n\n\n\n<p>Would you rather register with a paper form? Download, print, complete and deliver to the practice in person. <\/p>\n\n\n\n<p><a href=\"https:\/\/assets.genpra.com\/pdf\/PRF1-050723.pdf\">Register at our GP surgery with a paper form<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How to Register<\/h2>\n\n\n\n<ul>\n<li>After you have&nbsp;collected and&nbsp;completed the forms please bring them back to the surgery with 2 forms of identification (ID).<br>One of these must contain your address (e.g. utility bill, tenancy agreement, mortgage agreement, bank statement), and the other to have your photograph (eg. driving licence, passport, for children a birth certificate is adequate).<br>Until you have submitted your ID you will not be registered at the practice and cannot be seen for any treatment.<\/li>\n\n\n\n<li>Please provide&nbsp;your NHS number (10 digit number) on the registration form.&nbsp;This will help us trace your medical records more quickly.<\/li>\n\n\n\n<li>You will be allocated a specific GP who has vacancies on their list. If you have a preference please speak to the Patient Services staff member. However, if the doctors list is full, it may not be possible to accommodate your preference.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">New Patient Health Check<\/h2>\n\n\n\n<p>All new patients aged over 18 are invited to make an appointment for a routine health check with the GP.
